The patient was referred to Dr Ridhwan Ahmed with a long-standing sinus tract and persistent pus discharge associated with a heavily broken-down and decayed lower left first molar (LL6).
Root canal treatment had previously been attempted on two separate occasions by two different dentists, but due to the complexity of the tooth, the patient was subsequently referred to Dr Ridhwan for specialist endodontic treatment.
Why was this case complex?
The tooth presented with a severe 90-degree curvature near the end of the root, making the canals particularly challenging to negotiate safely. The patient also had limited mouth opening, making access to the tooth more difficult.
Treatment
Using advanced endodontic techniques, Dr Ridhwan was able to successfully navigate the severely curved root canal anatomy and complete the root canal treatment.
Despite the complexity of the case, treatment was successfully completed in a single appointment.

The follow-up X-ray also demonstrated complete healing around the roots. The dark area previously visible around the ends of the roots, representing infection and bone loss, had completely resolved.
